Our staff will do their best to let you know if our doctors are in- or out-of-network for your plan at the time your appointment is being scheduled, however, it is ultimately the patient’s responsibility to know their insurance plan.
Always bring your insurance card to your appointment.

To provide service to as many patients as possible ENT Associates is contracted with a wide array of insurance carriers, but not all. We currently DO NOT ACCEPT these insurance plans – click here. The list is subject to change, so please check back periodically.

AUTHORIZATIONS & REFERRALS
Be aware that many insurance plans require authorization or a referral for specialized medical care at our practice. It is your responsibility to contact your insurance company or primary care physician for more information and to obtain the necessary paperwork.
If a valid referral is necessary, but not obtained, you will either be asked to pay for your visit in full or to reschedule your appointment.
COPAYS & DEDUCTIBLES
All co-pays, deductibles, and coinsurances are collected at the time of your visit.
For diagnosis and treatment, patient examinations may require in-office tests and/or diagnostic procedures. Depending on your insurance coverage, these may require an additional co-payment or deductible. Please contact your insurance carrier to determine individual benefits.
Payment for self-pay appointments and services not covered by insurance will be expected at the time of service. Some instances will require payment prior to services being rendered. Patients are responsible for any unpaid balances on their accounts.
